Important information:
- Your business name as it appears on your state resale or sales tax license must be the same name used on the resale certificate form. If you operate as a DBA, please include this name as well
- We request that you provide the physical location of your business on the address line; we are unable to accept a PO box in this field
- Both the description of your business and the description of your product must be included in the required fields. New York customers, please be specific.
- The form must be hand signed and dated with the date the form is submitted; we are unable to accept a postdated form.
